This book is a medical fiction thriller taking place in an ER in Los Angeles, California and based on a bioterrorism attack resulting in an outbreak of Smallpox. Although the book is fiction, it is based on research by
Dr. Paulo J. Reyes and his 25 years of medical experience, as well as extensive medical research for his first book and his training as a First Responder for Terrorist attacks in California.

The book is very timely given the fact that the government acknowledges that another terror attack is a matter of if, and not when. The
government has also ordered 80 million doses of smallpox vaccination in case of a biochemical attack and the President himself and military have been vaccinated. Plus, in a recent article by Carla Johnson and posted
on www.WashingtonPost.com, it acknowledges that doctors in training miss bioterror diagnosis more than half the time on multiple-choice tests. It’s alarming to think what could happen if a case were to present itself
as it did with Anthrax in 2001.

In this book you journey through the massive panic of the outbreak, the initial diagnosis being made, the convincing of staff and higher ups of the diagnosis, and then the rush to obtain the cure. You grow fond of the characters and as each faces the threat of the disease, you are yearning for a quick cure to save them in time. It clearly portrays how rapidly the disease spreads, the potential devastation if air-borne, and the fatality
of those in contact. The book is written with great care and precision.

Paulo J. Reyes was born in the San Joaquin Valley and grew up in the foothills of the sierras. He attended the UCSF School of Medicine where he obtained his MD. He subsequently trained at UCLA. He has over 25 years experience in ER medicine and internal medicine and is a First Responder for the busiest cities in California.
He has firsthand disaster recovery experience and participated in
various disaster relief efforts in California including the Air Mexicana
crash in 1986, the last major earthquake in L.A. in 1994, and the
influenza epidemic 1997-1998.

He’s also the author of Health-Care Reform or redistribution of Cost? His research of the health care system has given him a unique insight into its shortcomings. Realizing that there exist a clear and present danger of a terrorist attack today leads him to agree with the proponent of voluntary smallpox vaccination. He himself has been vaccinated with smallpox and understands the difficulty of obtaining the vaccine.He is presently
working on a sequel to the book.

alike. What adds further drama to the story is the discovery that this particular form of smallpox is the particularly aggressive form -- sledgehammer smallpox, or malignant smallpox. Sledgehammer smallpox develops a non-typical rash and patients succumb to the disease fairly rapidly, many dieing within a day and a half. In the book they witness
that realize the urgency of the threat.

in the ER symbolize real events that occur in emergency rooms daily.
The author, Dr. Paulo J. Reyes, is a practicing Emergency Room Doctor
and First Responder to disasters in Los Angeles, California with a medical career spanning 25 years. His medical expertise helped to create a

Preparing for the next terrorist attack … Are we ready?
California, October, 2005. Intelligence experts believe that it is only a matter of time before there is another terrorist attack in the U.S.
The recent scares in New York and Baltimore and the deadly terrorist bombings in Bali are but a frightening indication as are the earlier attacks
in Egypt, London and Madrid. These attacks are aimed at striking terror among millions of people. Of the rising concerns is the fear that terrorists will use biological chemicals against us. Are we ready? It appears not.

A study listed in Archives of Internal Medicine showed 631 doctors,
mostly medical residents, were given a test prior to completing an online training course. On the pretest, half the doctors misdiagnosed botulism,
84 percent misdiagnosed plague and a case of routine chickenpox was misdiagnosed as smallpox by 42 percent of the doctors. "We've got a dangerous gap here and we need a much clearer strategic game plan,"
said Shelley Hearne, executive director of Trust for America's Health,
which tracks how well states are prepared for bioterrorism or pandemic.
In addition to the lack of bioterror knowledge in health care professionals, high-ranking officials in the US Health and Human Services Department
have stated major concerns about the government's ability to deliver medicines in the event of a bioterror attack. In response to these
concerns, steps are being taken to address these issues.

To improve the delivery of medicines in the event of a national emergency due to widespread illness caused by terrorist attacks with biological weapons, some suggested steps include the delivery of medicines from
local fire stations, police stations, and even via the US Postal Service.
In the book Sledgehammer, Dr. Paulo J. magnitude will require the cooperation of all nations. The United States must prepare emergency response strategies and develop effective means of delivering medicines to all parts of the country. Let’s prepare now so
our nation can respond rapidly and effectively resulting in minimal damage and loss of live.
